- MEDICAL INSURANCE
cost, doctor fees and contains a major medical provision. A
portion of the entire premium is deducted each week from your
paycheck to partially offset premium costs.
- HEALTH REIMBURSEMENT ACCOUNT
enrolled in the medical plan, N & M Transfer provides employee
with monthly funding on a Mastercard, to be used for medically
necessary out-of-pocket expenses.
- DENTAL INSURANCE
single or family coverage is provided at no cost to the employee.
- PRESCRIPTION DRUG COVERAGE
enroll in the medical insurance plan you will also receive
prescription drug benefits.
- LIFE INSURANCE
A $25,000 term life
insurance policy is provided at no cost to the employee. There is an
Accidental Death & Dismemberment (AD&D) benefit in which
case the amount would be doubled (subject to the policy rules).
Employees also have the option of purchasing additional term life
insurance coverage on themselves and any eligible dependents for a
weekly premium amount that is figured according to the insured's
- SHORT TERM DISABILITY
with a deduction equating to 50% of total premium calculated as a
percentage of your wages. Covers 60% of employee wages, up to a
maximum of $3,000 per month, when unable to work due to an injury or
illness occurring off-the-job.
- LONG TERM DISABILITY
takes effect when the maximum 26 weeks is exhausted on short term
policy. The premium deduction from employee paycheck is combined
with the cost of the short-term disability. Covers 60% of employee
wages, up to a maximum of $3,000 per month, when unable to work due
to an injury or illness occurring off-the-job.
- VISION INSURANCE
Single, Limited Family, or Family coverage is provided for a weekly
premium amount that is deducted off of the employee's paycheck
before taxes are calculated. Covers one eye exam and one pair of
glasses or contacts, per person, in a 12-month period. Covered in
full only if you see a doctor in the Provider Network.
- FLEXIBLE SPENDING
Employee has the
option annually to use tax-free dollars to help pay for medical,
vision, dental, over-the-counter, and dependant care expenses.
- N & M 401K PROFIT SHARING PLAN
After one full year of employment, you are eligible to participate
in the N & M 401K Profit Sharing Plan.
